Before Your Appointment

To help us provide the best possible care, please bring the following items to your appointment:

  • Current insurance card
  • Physician referral (if required by your insurance plan)
  • List of current medications, including dosage and frequency
  • Relevant medical records, test results, or imaging studies
  • Photo identification

Insurance & Payment Information

Texas Institute of Chest & Sleep Disorders participates with Medicare and most major insurance plans.

Please note:

  • Copayments, deductibles, and coinsurance amounts required by your insurance carrier may be due at the time of service.
  • Coverage and benefits vary by plan.
  • Patients are responsible for any charges not covered by their insurance provider.

Texas Institute of Chest & Sleep Disorders accepts Medicare and participates with many major insurance plans.

Because insurance networks and participation agreements may change, we encourage patients to contact our office before their appointment to verify coverage, benefits, referral requirements, and any out-of-pocket responsibilities.
