Physician referral (if required by your insurance plan)
List of current medications, including dosage and frequency
Relevant medical records, test results, or imaging studies
Photo identification

Texas Institute of Chest & Sleep Disorders participates with Medicare and most major insurance plans.
Please note:
Copayments, deductibles, and coinsurance amounts required by your insurance carrier may be due at the time of service.
Coverage and benefits vary by plan.
Patients are responsible for any charges not covered by their insurance provider.
